Dse Assessment for a lady with diagnosed tendonitus in the shoulder and RSI in the arm

Medical History
The client is suffering with diagnosed tendonitis in her shoulder and with a possible RSI in her arm and wrist which really needs to be properly diagnosed. It is likely that the symptoms experienced in the wrist are part of what would be termed a Cummulative Trauma Disorder. This means that the pain in the wrist is probably linked to the pain in the shoulder as the upper limbs are currently receiving poor support with no arms to rest on and this puts trauma on the vulnerable wrist area as the client uses the work surface to rest on.
The Client told me that her main areas of pain are the wrist, arm, across the shoulders and up the neck to the base of the skull. She has been using ibuprofen gel (which is an AI) and co-dydramol in tablet form which is an opiod that stimulates endorphins (the body’s natural pain killers).
Anatomical Measurements required
The Client is 5ft 8 tall sitting at a desk height of 720 mm which is within the scale measure for her height.
Her seat height to the top of the pad was 530 mm
Her seat depth was 470 mm with a width of 480 mm
The measurement from the back of her knee to the back of her buttock is 560 mm so the seat is a little on the small size for someone that has such long upper legs. Again the client measures 480 mm across the hips when seated so the chair is only just supporting her . Sitting to the extent or beyond the extent of a chair can restrict blood flow.
The keyboard was 15cm from the front of the desk to the front of the keyboard and I would always bring this forward as it means that the support for the upper limb is on the desk rather than closer to the torso.
The client is prone to tucking her legs underneath her, probably due to the seat pad being small, I have advised about a better working posture sat back into the chair.

I have recommended an Axia Pro Operator chair with 8N multifunctional arms for the client to trial. The Axia pro has unique pelvic support which will keep the client moving constantly in a dynamic working action rather than sitting in more static positions that can lead to static muscle strain. The arms on the chair are fully multifunctional and will adjust to support the client’s arms and help take the pressure off her shoulders , Most importantly we can use the chair to develop a better working posture where the body weight is supported sat back into the chair.

I would like the client to try using a docuglide which would help act as both an in-line document holder and a writing slope. It would mean that she would work in a straight line and when writing in front of the computer it will bring her neck up and sit her back in the chair more which will again take the strain off of her neck.

General Recommendations
I have recommended that client doesn’t do any lifting or indeed carrying with her right arm.
I have suggested using the footrest she has as a focus to make sure her feet stay out in front of her.
If after setting up the new chair we need to adjust the desk we can do so (height)
Rest is the best cure for RSI
Make sure when using the Orbit trackball that the wrist is straight on and that upper limbs are supported as close to the torso as possible.

DSE assessment for male who has undergone spinal surgery, including laminoctomy

I spoke to Client at length about the surgery that he has had and the pain that he has experienced in his back. The surgery has been successful in remedying the sciatica by freeing the trapped nerve root. As this surgery involves a mid-line incision it means that it is no longer possible to give ordinary central lumbar support to the patient using a standard chair and only two options or left open. The first is to use a split back chair which supports from the side of the back the other is to use a chair that supports purely from the pelvis such as a kneeling stool.
As we are being guided by the clients consultant to keep him more elevated and the pelvis forward to reduce the pressure around the lower discs, I believe we should allow the client to sit using a kneeling stool. I have recommended that he trials the Hag as it has good adjustment and re-acts more like a traditional VDU chair.

Dse Assessment For Slim Lady with Multiple Back Injuries

Client is returning to work after a very serious car accident.
Medical Details
Broken Pelvis 9 fractures. Right Femur (Pin in place in Rf)
also plating round pelvis and fusion of coccyx
Movement around pelvis and coccyx is restricted.
Leg lengths are different causing problems around pelvic floor and mis-alignment in lower lumbar.
Pelvis is out on the right.
I asked the client about insoles to help compensate and the Physio is looking into this.
Clients’s ribs were very bruised and her neck hurt with movement. She sustained whiplash injuries but as she had to lie still for 3 months the problems with her neck rectified themselves. However her neck muscles may now be quite weak.

As Client is quite short in the leg. She needs a footrest support. It is important that they has good spinal support. 15% of working week at PC doing drawings and learning packages for demonstration.
5 - 6 hours a week at home as well. - mainly drawing or CNC packages.
Pro - desk top etc..rehearse these at home for demonstrations.
Medical:
Diagnosed with Parkinsons - 3 years ago.
First symptoms tremor. Sleep depravation.
Some slight problem swallowing
Anatomical Measurements:
Back of buttock to back of knee length 520mm
Under knee to bottom of Foot 500mm
Back height to shoulder 660mm

After talking to the Client, I feel he needs a chair that fits him better to give him adequate support of the body but as his condition may change over the next few years I think that it would be better to re-assess his needs on an ongoing basis rather than spend a lot of money on a piece of equipment now that may not be suitable further down the line.
I understand that voice amplification for lectures and voice recognition software have been suggested. These are both good ideas although there is a bit of contradiction with this advice. As the client’s voice is very quiet you might find that the software doesn’t perform as well as it should and most of his time on the computer is spent in various CAD packages so it is these packages that the software needs to work with more than anything.
My recommendation would be a flame chair costing £189.00 + VAT which would give the client good thorasic support and arm support as well as helping to brace his pelvis properly. The extra support will enable the client to work in a more supported position which will help if his condition gets worse.
